測る道具の点検03 総合診療という領域/JAPANEUROPEOTHER

専門科の境界は、何が引いたのか

内科や外科は一見どこの国でも同じものを指すように見えるのに、総合診療だけが定まらない。なぜか。「道具が科を作った」という仮説を立て、文献で検証した。結果はそのままの形では成り立たなかった。医学史は1940年代以降この説を退けており、年表も逆を向いている——1839年のパリの専門誌には検眼鏡の12年前に「目の病気」が載っている。1862年の書評子は喉頭鏡について「専門科にする話ではない」と書いた。ただし「道具は科を作らなかった」も言いすぎで、道具はしばしば境界の軸や正当化として働いていた。分類が対象を作り、技術が実践可能性と正当化を与え、職業集団が管轄を要求し、制度が境界を固定する。

前回まで、この列は数字の来歴を辿ってきた。閾値がどこから来たのか、分母に誰が入っていないのか、標榜科の政令に総合診療という語が何回現れるか(0回だった)1※訂正。

今回は、読者からの問いに答える形で書く。なぜ総合診療だけが、世界的に見ても定義が定まらないのか。内科や外科なら、一見、どこの国でも同じものを指すように見える(前回見たとおり、実際には国際分類の側でも揺れている)。それらも昔はあやふやで、時代とともに揃えられてきたのだろうか。

私は仮説を立てた。そして、その仮説は否定された。以下はその記録である。

私が立てた仮説

専門科の境界は、道具が引いたのではないか。

検眼鏡(1851年)が眼科を、喉頭鏡(1854/55年)が耳鼻咽喉科を、X線(1895年)が放射線科を作った。道具は「ここから先は私の領分だ」という線を、物理的に引いてくれる。

そして総合診療には、固有の道具が無い。だから線が引けない。

この後半には、当事者による裏づけがある。カナダの家庭医療学者マクウィニーが、1996年の講演でこう述べている2。

I began by saying that general practice has no technology to call its own.

同じ講演の冒頭で、前半にあたることも言っている。

Other fields define themselves in terms of content: diseases, organ systems or technologies.

他の分野は content で自分を定義する——疾患、臓器系、あるいは技術で。仮説はきれいに立った。

医学史は、この説を1940年代から退けてきた

まず結論から書く。

While the earliest work on this topic, George Rosen’s sociologically informed The specialization of medicine (1944), highlighted the role of medical theory in specialty formation, historians of medicine since the 1940s have overwhelmingly argued that medical specialization is better characterized as an economic, social or political process.

英国スポーツ医学の成立を扱った論文の一節である3。道具説は通説ではなく、80年かけて退けられてきた最初期の説だった。

専門分化の比較史を書いたワイズは、自分の論文の中で明示的にこう書いている4。

I do not, as they did, attribute primary responsibility to the rise of organic localism and then new technologies. These factors, at most, provided an axis along which certain specialties were able to develop.

「せいぜい、軸を提供したにすぎない」。

同じ著者の比較史の本は、専門分化の第一義的な正当化を、こう置いている5。

gained its initial and primary justification as a form of knowledge production and dissemination rather than as a type of skill or form of practice

技能や実践の型ではなく、知識生産の様式として正当化された。道具の操作技能は、はっきり後者に属する。

そして彼が代わりに置いた原因は、二つだった。一つは知識を拡張したいという集団的な欲求。もう一つが——

administrative rationality suggested that one could best manage large populations through proper classification, gathering together individuals belonging to the same class and separating those belonging to different categories

行政的合理性。大きな集団は、適切に分類することで最もよく管理できる。この語はあとでもう一度出てくる。

年表が、逆を向いていた

仮説を殺したのは、抽象論ではなく年表だった。

1839年、パリの専門誌 L’Esculape の題字に、専門領域が15列挙されている4。その中に、こうある。

Maladies des yeux(目の病気) Maladies des oreilles(耳の病気)

検眼鏡の12年前、喉頭鏡の16年前である。

眼科の年表を並べると、道具の位置がわかる。

(1839年の題字が示すのは、眼疾患が検眼鏡以前から専門領域として認識されていたことであって、専門医制度があったことではない。)

この段階で見ても、道具は先頭ではなく途中に現れる。

ワイズはさらに踏み込んでいる4。

even apparently organically based specialties like ophthalmology or urology predate pathological anatomy and are based on specific surgical procedures, couching cataracts or surgery for the stone

眼科も泌尿器科も、病理解剖学より古い。もとになっているのは白内障の手術と結石の手術——器械ではなく、手技である。

X線の例も置く。1895年の発見から American Board of Radiology の設立(1934年)まで39年かかっている。

ただし公開時の本文は、これを「39年間、機械を握っていたのは写真師だった」と読んだ。二つの異なる話を接続している※訂正。1934年は放射線科の成立年ではなく専門医認定機関の設立年であり、1900年には米国初の放射線学会が成立している。そして写真師が撮っていたのは主に非医療用途のX線である——ペンシルヴェニア病院では1897〜1909年に chief medical resident が機械を担当し、1912年には専任医と部門が成立している6。

Many people … didn’t buy a medical X-ray; it wasn’t being taken by physicians. Instead, it was being taken by photographers.

この事例が示すのは、機械の発明だけでは専門職の管轄は決まらず、部門・報酬・解釈を担う専門家が必要だったということである。

同時代の人が、専門科化に反対している

喉頭鏡の普及から4年後、1862年の書評論文にこうある7。

The subject is not one to be made another specialty of. The unprofessional public need not believe that there is only one man who is capable of diagnosing diseases of the larynx or looking down the trachea.

with a few precautions, any person of ordinary dexterity may speedily acquire the tact necessary for the employment of the instrument.

「誰でもすぐ使えるようになる。専門科にする話ではない」。

それでも耳鼻咽喉科は成立した。現代の研究は、実効的な成立を1880年代に置いている——喉頭鏡から25年後、局所麻酔で喉頭を手術できるようになった時点である8。

ここは「道具以外の理由で」とは書けない※訂正。局所麻酔と手術技術が喉頭鏡に結びついた時点であり、むしろ技術の役割を示している。1862年の反対論が示すのは、「道具が専門科を自動的に生んだわけではない」「管轄権が争われた」というところまでである。

ついでに言えば、laryngology という語のほうが喉頭鏡より約100年古い。18世紀にフランス語で現れている8。道具が分野を名づけたのでもなかった。

では、道具は何をしていたのか

何もしていなかったわけではない。使われた場面が、特定できる。

1870年代、パリ医学部で講座を新設するかどうかの議論があった。そのときの記録に、ワイズはこう注記している4。

At the head of the list was ophthalmology: academic reports and debates referred specifically to the discoveries that the ophthalmoscope had made possible.

科を生んだのではない。既にある科が、講座という椅子を要求するときの論拠になった。脚注では、耳鼻咽喉科医たちが「新しい器械に基づく専門化のモデル」を意識的に踏襲したことも記録されている。

この区別を、最も簡潔に言い切っているのが先の論文である3。

道具は原因ではなく、正当化の側にあった。

聴診器という、未解決の問い

私は当初、仮説の弱点として聴診器を置いていた。1816年の聴診器は19世紀最大の診断器具の一つでありながら、「聴診科」を生まなかった。この非対称は説明できるのか。

説明した研究は、見つからなかった。ただし、見つからなかった以上のことが分かった。職業的な医学史家が、まったく同じ問いを立てて、未解決だと書いている9。

Why, for example, earlier stethoscopists should not have become specialists, while ophthalmoscopists, many of whom worked on nervous diseases, should have done so, is a curiosity that still requires further contemplation.

論文の脚注82である。「なお熟考を要する奇妙な点」。

私が用意していた説明のうち、一つは既に反証されている。「聴診器は習得が容易だったから」という説明は、逆向きの証拠を持つ——検眼鏡のほうが医師の抵抗が少なかったという研究がある10。

そして、聴診器の領域が最終的に専門科になったときの経緯が残っている。英国の心臓病学である。第一次世界大戦の「兵士の心臓」問題から始まったこの過程を追った研究は、こう書いている11。

The fourth basis for speciality formation in Rosen’s model, improved technology, appears, surprisingly, to have played little part in Lewis’s plan.

それどころか、当事者たちは器械から離れる方向に動いていた。

Physicians at Hampstead and Colchester directed attention away from local, particular valvular defects, de-emphasized the importance of murmurs and the stethoscope, and emphasized the global working ability of the heart.

If the clinical approach to breathless soldiers was to be easily incorporated into the contemporary medical system, it needed an easily appreciable, non-instrumental basis.

器械に依らない基盤が必要だった。そして、その集団を専門科の核にしたものは何だったか。同じ論文が、身も蓋もなく書いている。

if the club had no unity of research, technology, or practice, was the Cardiac Club itself a speciality group? … this case study demonstrates how a group of physicians bound by social ties can act as the nucleus for later speciality formation.

道具を持たない科は、どう境界を引いたか

ここからが、総合診療にとって直接の材料になる。

小児科は、総合診療とまったく同じ困難を抱えていた12。

Their two main dilemmas were that, firstly, paediatrics was not an organ based specialty like, for example, ophthalmology and, secondly, the management of children’s diseases had always been practised by other general physicians. As a result paediatrics had difficulty in demarcating itself from general medicine.

臓器を持たない。そして、その仕事は既に一般医がやっていた。一言一句、総合診療の話に置き換えられる。

解き方も記録されている。

If development is the territory of paediatrics, the problem of not being a single organ specialty is solved.

「発達」という領域を発明した。臓器の代わりに、時間軸を取った。

老年医学は、成立の過程が一次史料で追える。マージョリー・ウォレンが1943年の BMJ に書いた記事である13。

In 1935, by the appropriation of the Poor Law institution, the West Middlesex County Hospital inherited overnight several hundreds of chronic sick patients, unclassified and ill-assorted. Early in 1936, still overwhelmed by the problem of the proper care and treatment of these patients, I interested myself in a scheme of classification

そして同じ記事の別の見出しの下で、こう要求する。

until the subject is recognized as a special branch of medicine in this country it will not receive the sympathy and attention it deserves.

施設の接収(1935年)→ 患者の分類(1936年)→ 専門科の要求(1943年)。この記事に、診断機器は一つも出てこない。

仕上げたのは制度だった14。

as a thriving and influential medical specialty geriatric medicine was essentially a product of the British NHS

そして最も効く反例が、スポーツ医学である3。

It has virtually no unique diseases or injuries (one can get tennis elbow cleaning floors) nor any unique treatment modalities or technologies.

固有の病気も、固有の治療法も、固有の道具も無い。それでも2005年に、英国の正式な専門科になった。

分類が、道具だった

こうして並べると、繰り返し出てくるものが一つある。

ワイズの proper classification。ウォレンの a scheme of classification。小児科の「発達」という領域。老年医学の「生理的老年」。

選んだ事例では、専門科を創設する行為に、いずれも集団を仕分ける作業が含まれていた。

ただし「分類が作った」もまた、証明されていない※訂正。分類はどの専門科にも事後的に見つけられるので、それだけでは原因と相関を区別できない。道具説という単因論を退けた先で、分類説という別の単因論に置き換えることはできない。

本文の材料から言えるのは、次の三層である。

  1. 分類が患者・疾患・行為を括る
  2. 集団や組織がその領域を管轄すると主張する
  3. 講座・病棟・学会・研修・認定・法令が境界を固定する

「分類が道具だった」は、以下では私の仮説として書く。文献が証明した結論ではない。

この列の題は「測る道具の点検」だが、点検すべき道具は、器械よりもむしろ分類そのものだということになる。第5回で見た医療法施行令の四区分——部位・年齢性別・処置・疾患——は、まさにその道具の現物だった。

そして四区分はいずれも「切り出し方」である。総合診療は、その四つの切り出しを全部拒否することで定義されている。だから切り出しの語彙で名づけられない。

日本の政令に総合診療が無いのは、日本の制度が特殊だからではない。「切り出さない」を、切り出しの体系で表現できないからである。

内科と外科の分離も、最初は知の区分ではなかった

読者の問いの後半——内科や外科も昔はあやふやだったのか——にも答えておく。

英国法に「医師」という統一カテゴリーが現れるのは、1858年である15。

The general concept of the qualified or registered practitioner had no place in English law prior to the Medical Act of 1858; instead there were separate laws relating to physicians, to surgeons, and to apothecaries.

それ以前にあったのは、身分ごとの別々の法だった。そして身分差の中身は、知識ではなかった。

In sharp contrast to physicians, surgeons were regarded as craftsmen rather than gentlemen.

さらに、内科の職域が縮んだ理由が興味深い。外から削られたのではなかった。

the disdain which physicians, as a body of learned men, felt for manual work, had led to a contraction in their duties.

physicians が手仕事を身分的に忌避したので、自ら職域を縮小させた。

ここで訳語に注意が要る※訂正。18世紀英国の physician / surgeon / apothecary は、現代の内科/外科という専門科というより、法的身分・教育・職能の区分に近い。「内科医が自分で職域を縮小した」と訳すと時代錯誤に読める。残余は、身分秩序の内側からの忌避の結果だった。

境界が実際にどう決まったかの実例も残っている。19世紀末、婦人科がどちらのものかで争いが起きた16。

the root of it is the argument made by surgeons that gynaecology belonged to them because it was operative; the physician-accoucheurs claimed that obstetrics and gynaecology belonged together. As we know, the surgeons won the argument.

論争で決まった。知識の性質からの必然ではなく。

したがって「内科や外科は昔から定義が揃っていた」わけではない。揃ったように見えるのは、身分の区分が先にあり、知的定義が数百年かけてあとから追いついたからである。

一般医だけが、その身分秩序のどこにも入れなかった15。

the general practitioner was neither physician, nor surgeon, nor apothecary … Unwanted by all the medical corporations, the general practitioner was at best tolerated, never welcomed as a full member of the professional community.

英国で一般医のカレッジが作られたのは1952年だが、その時点の記録はもっと直接的である17。

general practice had not even been defined – therefore, it was not then a discipline.

So hostile to the idea of a generalist college were the then three medical royal colleges that the College of General Practitioners had to be founded in secret.

団体が先にでき、定義はそのあと数十年かけて作られた。しかも設立は、秘密裏に行われた。

医制では、科は免状に書き込む属性だった

日本の出発点を見ておく。医制(1874年)全76条を機械走査した18。(以下で分かるのは、この全文を確認した範囲で標榜を定める条項を見いだせなかった、ということである※訂正。OCRで特定の語が0件だったことは有力な手がかりだが、法的概念が存在しなかったことの完全な証明ではない。)

これは語彙が無かったからではない。同じ『法規分類大全』衛生門の別の箇所には、こういう条文が実在する。

産婆營業者ハ左式ノ看板ヲ戶外ヘ揭出スヘシ

産婆には看板を義務づけ、医師には課さなかった。

では医制で「科」は何だったか。仮免状の書式雛形の割注に、こうある。

何科トハ内、外、眼、産、小兒、整骨、口中、ノ七科ヲ云フ

少なくとも仮免状の書式では、科は免状に書き込む属性だった※訂正。外に向かって掲げる表示ではない。

科が現れるのは二箇所だけ——免許の要件(第三十七条)と、診察料の定め方(第四十一条)である。この文書の中に、標榜を定める条項は見いだせなかった。(「日本では科はそもそも看板ではなかった」まで広げるには、他の免許・営業・広告制度も確かめる必要がある。)

標榜科が法律に列挙されるのは、戦後の医療法(1948年)を待つ。制定時の条文は第四十条で、別名を含めて16種だった19。

(通説では第七十条とされるが、それは1950年以降の話である。医療法人の章がまるごと挿入され、条番号が32ずつ繰り下がった結果だった。)

総合科は提案され、部会は17年半開かれなかった

日本にも、境界を引き直そうとした瞬間がある。

2007年5月、医道審議会の標榜部会に、こういう資料が出ている20。

総合的な診療能力に関する医療に関する診療科として総合科を新設する

部会は第1回から第5回まで、2007年5月から2008年2月にかけて開かれた。そして2008年2月の政令に、総合科は入らなかった。

次に部会が開かれたのは、2025年9月4日である。17年半、この部会は開かれなかった※訂正。(「総合科が17年半置かれた」と書くのは広すぎる。その間に総合診療専門医制度は動いている。確認できたのは診療科名標榜部会が開かれなかったという事実である。)

再開後に処理された最初の改正は、総合診療科ではなく睡眠障害の追加だった(2026年6月1日施行)。

いま、体系は二つに割れている。同じ厚生労働省の医療広告ガイドラインの中で21——

項目認否
「総合診療専門医」という資格の広告認められる(基本的な診療領域に「総合診療」が明記)
「総合診療科」という診療科名の広告通常の広告可能な診療科名としては認められない(法令に根拠がない)※訂正

(ただし例外がある。厚生労働省のQ&Aは、広告可能事項の限定解除の要件を満たすウェブサイト等であれば表示しうるとしている。)

そして19の基本領域のうち、総合診療専門医だけが基幹学会を持たない。専門医機構自身の資料に、こうある22。

関連学会の名称/学会員数:学会がないため、会員数は0人

この列にとって

仮説は、そのままの形では成り立たなかった。ただし「道具は科を作らなかった」という断定も、引用文献が支持する範囲を超えている※訂正。ワイズ自身が技術について “provided an axis along which certain specialties were able to develop” と書いており、ヘギーも、専門科の形は知識・技術・制度が決め、専門分化を駆動するのは経済・社会・政治的要因だと整理している。

書ける範囲はこうである。道具は専門科の必要条件でも十分条件でもなく、しばしば境界の軸や、制度化を求めるときの正当化として働いた。

だから「総合診療に固有の道具が無いから定義が定まらない」も、単独の説明としては成り立たない。

マクウィニーの “no technology to call its own” は本物の一次資料だが、位置が変わる。分野の自己認識としては真だが、定義が定まらない原因としては、おそらく間違っている。

この記事の材料から最も強く言えるのは、単因の説明ではない※訂正。

専門科の境界を引いたのは、道具でも分類でもない。分類が対象を作り、技術が実践可能性と正当化を与え、職業集団が管轄を要求し、制度がその境界を固定した。

眼科も、放射線科も、小児科も、老年医学も、スポーツ医学も、日本の標榜制度も、この四段階に収まる。そして総合診療について言えば、第一段階でつまずいている——切り出しの語彙で括れない。それは第5回と第6回で辿った場所そのものだった。政令に名前が無いこと、国際統計の列が空いていること。

そしてもう一つ、書いておかなければならないことがある。分類されないことは、記述されないことに直結する。262研究・25か国のシステマティックレビューが、こう報告している23。

Ninety studies defined generalists in deficit terms—as physicians lacking specialist training. Only 23 described positive attributes of generalism…

This discourse positions generalism as ‘a lack of’ rather than a specific way of practice.

90研究が「専門研修を欠く医師」という欠損の形で定義し、肯定的な属性で記述したのは23研究だけ。そして262研究のうち122研究(47%)は、そもそも定義していない。

第6回で見た OECD の空欄と、同じ形をしている。欠けているのではなく、欠けとして定義されている。

この列が準備しようとしているのは、何を測るかの合意だった。その手前に、何を一つの類として括るかという合意がある。分類が道具である以上、点検の対象はそこまで遡る。

小児科は「発達」を発明した。老年医学は「分類の仕組み」から始めた。どちらも、器械が先に来たのではなかった。そして両方とも、そのあとに職業集団の要求と制度の固定が続いている。


この記事で最も確信度が低い箇所を明記しておく。

①「道具説の元祖は Rosen(1944) である」という枠は、原典を読めていない。HathiTrust が検索のみの公開で、本文に到達できなかった。この枠は Weisz と Howell という二次的な要約2件の上に立っている。

②「分類が道具だった」は、私の言い換えである。文献はそれぞれの文脈で classification に触れているが、「分類こそが専門科を作る道具だ」と総括した研究を確認したわけではない。

③ 精神科を扱わなかったのは、「収容施設が先にあった」という説明が現在も係争中で、断定できる状態にないためである。

④ Weisz の書籍本文には到達できていない。引用は査読誌の書評経由である。

訂正 CORRECTIONS

公開後に直したこと。本文は直したうえで、何をどう変えたかをここに残している。本文中の※ が、直した箇所を指す。

  1. 訂正「道具は科を作らなかった」という断定を、引用文献の範囲に戻した2026-09-01 · douguワイズ自身が技術について "provided an axis along which certain specialties were able to develop" と書いており、ヘギーも専門科の形は知識・技術・制度が決め、専門分化を駆動するのは経済・社会・政治的要因だと整理している。「道具は専門科の必要条件でも十分条件でもなく、しばしば境界の軸や、制度化を求めるときの正当化として働いた」に改めた。
